
Social Worker - Perinatal Hospice/Bereavement
At Highmark Health, we are looking for a compassionate and dedicated Social Worker to join our Perinatal Hospice/Bereavement team. As a Social Worker, you will be a crucial member of the team, providing essential support to families facing difficult and heartbreaking circumstances. You will have the opportunity to make a positive impact on the lives of those in our care while providing effective interventions and resources to help them through their journey.The ideal candidate will have a Master’s Degree in Social Work, be a licensed Social Worker in the state of Pennsylvania, and have at least three years of experience in perinatal hospice/bereavement. A demonstrated commitment to patient-centered, holistic care and an understanding of the grief process are essential to this role. Responsibilities:
- Provide compassionate and effective support to families facing difficult and heartbreaking circumstances.
- Facilitate patient-centered, holistic care and an understanding of the grief process.
- Develop and implement appropriate interventions and resources to help patients through their journey.
- Collaborate with other members of the perinatal hospice/bereavement team to ensure the best possible care and outcomes.
- Remain current with best practices and social work standards.
- Attend and participate in required trainings and professional development opportunities.
- Maintain all required licensure, certifications, and other credentials.
- Adhere to all relevant laws, regulations, policies, and procedures.
- Document patient care activities and other relevant information accurately and in a timely manner.
