Job Description

At Highmark Health, we are looking for a compassionate and dedicated Social Worker to join our Perinatal Hospice/Bereavement team. As a Social Worker, you will be a crucial member of the team, providing essential support to families facing difficult and heartbreaking circumstances. You will have the opportunity to make a positive impact on the lives of those in our care while providing effective interventions and resources to help them through their journey.The ideal candidate will have a Master’s Degree in Social Work, be a licensed Social Worker in the state of Pennsylvania, and have at least three years of experience in perinatal hospice/bereavement. A demonstrated commitment to patient-centered, holistic care and an understanding of the grief process are essential to this role. Responsibilities:

  1. Provide compassionate and effective support to families facing difficult and heartbreaking circumstances.
  2. Facilitate patient-centered, holistic care and an understanding of the grief process.
  3. Develop and implement appropriate interventions and resources to help patients through their journey.
  4. Collaborate with other members of the perinatal hospice/bereavement team to ensure the best possible care and outcomes.
  5. Remain current with best practices and social work standards.
  6. Attend and participate in required trainings and professional development opportunities.
  7. Maintain all required licensure, certifications, and other credentials.
  8. Adhere to all relevant laws, regulations, policies, and procedures.
  9. Document patient care activities and other relevant information accurately and in a timely manner.
