Job Description

  1. Engage with community members and establish trusting relationships to effectively promote health and wellness initiatives.
  2. Conduct outreach and identify individuals and families in need of support and resources.
  3. Collaborate with healthcare providers and community organizations to coordinate and facilitate access to healthcare services.
  4. Educate community members on health-related topics such as disease prevention, healthy lifestyle choices, and healthcare options.
  5. Conduct assessments to identify and address the needs of community members, including social determinants of health.
  6. Develop and implement personalized care plans for individuals and families, in collaboration with healthcare providers.
  7. Monitor progress and provide ongoing support to individuals and families to ensure successful outcomes.
  8. Maintain accurate and up-to-date records of all community interactions and services provided.
  9. Act as a liaison between community members and healthcare organizations to improve communication and access to care.
  10. Stay informed on current community health issues and trends, and make recommendations for program improvements.
  11. Participate in community events and activities to raise awareness and promote health and wellness initiatives.
  12. Adhere to all ethical and legal standards, including maintaining confidentiality of all personal and medical information.
  13. Continuously seek opportunities to expand knowledge and skills in the field of community health.
  14. Collaborate with team members to develop and implement strategies for improving community health outcomes.
  15. Represent Humana in a positive and professional manner at all times.

Job Qualifications
  • Excellent Communication And Interpersonal Skills.

  • Bachelor's Degree In Public Health, Community Health, Or A Related Field.
