  1. Conduct assessments to determine the behavioral health needs of Medicaid members.
  2. Develop personalized care plans for each member, taking into consideration their specific needs and goals.
  3. Coordinate and collaborate with other healthcare professionals and community resources to ensure members receive comprehensive care.
  4. Advocate for members and their families to ensure they receive the best possible care and resources.
  5. Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
  6. Educate members and their families about behavioral health conditions, treatment options, and resources available.
  7. Maintain accurate and timely documentation of member interactions and progress.
  8. Stay up-to-date on Medicaid policies and regulations to ensure compliance and provide guidance to members.
  9. Conduct home visits and attend appointments with members to provide support and ensure their needs are being met.
  10. Collaborate with the interdisciplinary team to identify and address any barriers to care for members.
  11. Provide crisis intervention and support for members when needed.
  12. Participate in trainings and meetings to continuously improve knowledge and skills related to behavioral health.
  13. Adhere to ethical and professional standards in all interactions with members, colleagues, and partners.
  14. Maintain confidentiality and privacy of member information at all times.
  15. Serve as a resource and mentor for other team members.

Job Qualifications
