  1. Provide compassionate and comprehensive social work services to Humana members in their homes.
  2. Conduct thorough assessments of members' social, emotional, and psychological needs.
  3. Develop individualized care plans and goals in collaboration with members, their families, and other healthcare professionals.
  4. Educate members on available community resources and assist in accessing them.
  5. Advocate for members' rights and ensure their needs are met.
  6. Monitor and evaluate members' progress and make necessary adjustments to care plans.
  7. Collaborate with interdisciplinary teams to ensure coordinated and holistic care for members.
  8. Document all interactions and interventions accurately and timely.
  9. Maintain confidentiality and adhere to ethical and professional standards.
  10. Continuously stay updated on industry trends and best practices in medical social work.
  11. Participate in training and professional development opportunities to enhance knowledge and skills.
  12. Adhere to all company policies and procedures.
  13. Communicate effectively and professionally with members, families, and colleagues.
  14. Demonstrate a high level of empathy, sensitivity, and cultural competence in all interactions.
  15. Provide emotional support and counseling to members and their families.
  16. Facilitate support groups and educational sessions for members.
  17. Maintain a positive and collaborative working relationship with community partners and resources.
  18. Meet productivity and quality standards set by the company.
  19. Participate in performance improvement initiatives.
  20. Attend and actively participate in team meetings and case conferences.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work From An Accredited University

  • Current State Licensure As A Clinical Social Worker (Lcsw) Or Licensed Master Social Worker (Lmsw)

  • Minimum Of 2-3 Years Experience In Medical Social Work, Preferably In A Home Health Setting

  • Knowledge Of Medicare And Medicaid Regulations And Experience In Conducting Psychosocial Assessments And Developing Care Plans

  • Strong Communication And Interpersonal Skills, With The Ability To Work Effectively With Patients, Families, And Interdisciplinary Healthcare Teams.
