  1. Conduct initial evaluations and assessments of patients' physical abilities and limitations.
  2. Develop and implement individualized treatment plans based on patients' needs and goals.
  3. Provide ongoing therapy sessions and monitor progress to ensure optimal outcomes for patients.
  4. Collaborate with other healthcare professionals, such as doctors and nurses, to coordinate care and provide comprehensive treatment.
  5. Educate patients and their families on exercises and techniques to promote healing and prevent further injury.
  6. Document all patient interactions, progress, and treatment plans accurately and in a timely manner.
  7. Maintain a high level of professionalism and empathy while working with patients and their families.
  8. Stay current with industry developments and advancements in physical therapy techniques.
  9. Adhere to all ethical and legal standards in regards to patient care and confidentiality.
  10. Travel to patients' homes to provide therapy services as needed.
  11. Communicate effectively with patients, their families, and other healthcare professionals to ensure coordinated care.
  12. Monitor and order necessary equipment and supplies for therapy sessions.
  13. Participate in team meetings and contribute to the development of best practices and procedures.
  14. Adhere to all safety protocols and procedures to ensure a safe working environment for both patients and staff.
  15. Continuously strive to improve and enhance the quality of care provided to patients.

Job Qualifications
  • Bachelor's Degree In Physical Therapy: A Minimum Of A Bachelor's Degree In Physical Therapy From An Accredited Institution Is Required For This Position. This Ensures That The Candidate Has The Necessary Knowledge And Skills To Provide Effective Therapy Services.

  • State Licensure: All Candidates Must Hold A Valid State License To Practice As A Physical Therapist. This Demonstrates That The Therapist Has Met The Necessary Requirements And Has Been Approved To Practice By The State Regulatory Board.

  • Cpr Certification: As A Healthcare Professional, It Is Essential For A Physical Therapist To Have Current Cpr Certification. This Ensures That They Are Prepared To Handle Any Emergency Situations That May Arise While Providing Therapy Services.

  • Previous Experience In Home Health: Humana Is Seeking Candidates With Previous Experience In Home Health Settings. This Shows That The Therapist Is Familiar With The Unique Challenges And Requirements Of Providing Therapy Services In A Patient's Home.
