- Provide personalized and high-quality physical therapy services to patients in their homes.
- Assess patients' needs and develop individualized treatment plans to improve their physical function and independence.
- Communicate effectively with patients, their families, and other healthcare professionals to ensure coordinated and comprehensive care.
- Maintain accurate and timely documentation of patient progress and treatment plans.
- Utilize rehabilitative techniques to help patients achieve their treatment goals and improve their quality of life.
- Educate patients and their families on proper techniques for managing their condition and preventing further injury.
- Monitor and report any changes in patients' conditions to the appropriate healthcare team members.
- Uphold ethical and professional standards in accordance with the American Physical Therapy Association (APTA) guidelines.
- Participate in ongoing professional development and stay updated on current trends and techniques in physical therapy.
- Adhere to all company policies and procedures to ensure the highest level of patient care and safety.
- Collaborate with other healthcare professionals to develop and implement comprehensive care plans for patients.

Bachelor's Degree In Physical Therapy: A Minimum Of A Bachelor's Degree In Physical Therapy Is Required For This Position. This Qualification Ensures That The Candidate Has A Strong Foundation In The Principles And Techniques Of Physical Therapy.
State Licensure: The Candidate Must Possess A Valid State License To Practice As A Physical Therapist. This Ensures That The Candidate Is Qualified To Provide Physical Therapy Services In The State Where The Job Is Located.
Previous Experience In Home Health: Prior Experience In Home Health Is Preferred For This Position. This Experience Demonstrates The Candidate's Ability To Work Independently And Provide Quality Care In A Home Setting.
Strong Communication And Interpersonal Skills: Excellent Communication And Interpersonal Skills Are Essential For A Physical Therapist, Especially In A Home Health Setting Where They Will Be Interacting With Patients And Their Families On A Regular Basis.
Knowledge Of Medicare Guidelines And Regulations: Familiarity With Medicare Guidelines And Regulations Is Important For This Position As Humana Is A Medicare Advantage Provider. This Knowledge Will Ensure That The Physical Therapist Is Able To Provide Services In Compliance With Medicare Regulations.
