  1. Conduct initial evaluations and ongoing assessments to determine the appropriate speech therapy needs of patients.
  2. Develop individualized treatment plans based on patient needs and goals.
  3. Provide speech therapy services to patients in their homes, adhering to all regulatory and ethical standards.
  4. Collaborate with interdisciplinary team members, including physicians, nurses, and other therapists, to ensure comprehensive and coordinated care for patients.
  5. Utilize evidence-based practices and techniques to improve patient outcomes and functional abilities.
  6. Educate patients and their families on speech therapy exercises and techniques to improve communication and swallowing skills.
  7. Monitor and document patient progress, adjusting treatment plans as needed.
  8. Communicate effectively with patients, families, and caregivers to foster a positive therapeutic relationship.
  9. Maintain accurate and up-to-date patient records and documentation in accordance with organizational policies and procedures.
  10. Participate in continuing education and professional development opportunities to stay current on best practices and advancements in the field.
  11. Adhere to all safety and infection control protocols to ensure a safe and healthy environment for patients and staff.
  12. Maintain confidentiality and professionalism in all interactions with patients, families, and colleagues.
  13. Represent the organization in a positive and professional manner at all times.
  14. Continuously evaluate and improve own performance to provide the highest level of care to patients.
  15. Adhere to all organizational policies and procedures, including those related to patient care, documentation, and ethical standards.

Job Qualifications
  • Master's Degree In Speech Language Pathology: A Minimum Of A Master's Degree In Speech Language Pathology Is Required For This Position. This Ensures That The Candidate Has A Strong Foundation In The Principles And Techniques Of Speech And Language Therapy.

  • Certificate Of Clinical Competence In Speech Language Pathology (Ccc-Slp): The Candidate Must Hold A Valid Ccc-Slp Certification From The American Speech-Language-Hearing Association (Asha). This Certification Demonstrates That The Candidate Has Met The Rigorous Standards For Clinical Practice And Has Completed A Supervised Clinical Fellowship.

  • State Licensure: The Candidate Must Hold A Valid License To Practice As A Speech Language Pathologist In The State Where They Will Be Providing Services. This Ensures That The Candidate Has Met The State-Specific Requirements For Education, Training, And Clinical Experience.

  • Experience In Home Health: The Ideal Candidate Will Have Previous Experience Working As A Speech Language Pathologist In A Home Health Setting. This Experience Demonstrates The Candidate's Ability To Work Independently, Manage A Caseload, And Provide High-Quality Care To Patients In Their Homes.

  • Excellent Communication And Interpersonal Skills: As A Speech Language Pathologist, Strong Communication Skills Are Essential. The Candidate Must Be Able To Effectively Communicate With Patients, Their Families, And Other Healthcare Professionals. They Must Also Have Strong Interpersonal Skills To Build Rapport With Patients And Establish A Trusting Relationship.
