  1. Conduct telephonic assessments and provide ongoing support for members seeking behavioral health care services.
  2. Collaborate with a multidisciplinary team to develop individualized care plans for members.
  3. Utilize clinical expertise to provide guidance and support to members in managing their mental health and well-being.
  4. Assist members in navigating the complexities of the behavioral health care system, including finding appropriate providers and resources.
  5. Monitor and track member progress and outcomes, and make necessary adjustments to care plans as needed.
  6. Maintain accurate and timely documentation of all interactions with members.
  7. Communicate effectively and empathetically with members, demonstrating compassion and understanding.
  8. Stay up-to-date on current research and best practices in the field of behavioral health.
  9. Adhere to all ethical and legal standards in the provision of care.
  10. Collaborate with community resources and organizations to support members' overall well-being.
  11. Attend regular trainings and meetings to enhance knowledge and skills.
  12. Maintain confidentiality and adhere to HIPAA regulations in all interactions with members.
  13. Demonstrate a commitment to providing high-quality, patient-centered care.
  14. Continuously evaluate and improve processes and procedures to ensure the best possible outcomes for members.
  15. Serve as an advocate for members, ensuring their needs are met and their voices are heard.

Job Qualifications
  • Bachelor's Degree In Social Work, Psychology, Or Related Field

  • Minimum Of 3 Years Experience In Behavior Health Care Management

  • Knowledge Of Evidence-Based Treatments For Mental Health And Substance Abuse Disorders

  • Ability To Communicate Effectively And Professionally Over The Phone
