Complex Case Manager

We are seeking a dynamic and experienced Complex Case Manager to join our team at Kaiser Permanente. This individual will be responsible for managing cases for members with complex clinical and social needs and will ensure that members and their families receive the highest quality of care. Our ideal candidate will have strong assessment, interpersonal, and communication skills with a commitment to providing excellent patient care.This position requires a Bachelor’s degree in Nursing or Social Work, or a related field. A minimum of two years of experience in case management or a related field is also required. Our Complex Case Manager must be a self-starter with the ability to work both independently and collaboratively. An understanding of quality improvement, risk management, and utilization management is also essential.
