
Inpatient Pharmacist

- Dispense and review medications for inpatient hospital patients, ensuring accuracy and adherence to physicians' orders and established protocols.
- Collaborate with physicians, nurses, and other healthcare professionals to develop and implement individualized treatment plans for patients.
- Conduct medication reconciliation to ensure patients' medication histories are accurately documented.
- Monitor patients' responses to medications and make appropriate changes as needed.
- Educate patients and their families about proper medication use, potential side effects, and drug interactions.
- Ensure compliance with all state and federal laws and regulations related to dispensing and handling medications.
- Maintain accurate and up-to-date patient records, including medication profiles and treatment plans.
- Stay current on new medications, dosage forms, and treatment guidelines through continuous education and training.
- Collaborate with pharmacy technicians to ensure timely and accurate medication preparation and distribution.
- Participate in quality improvement initiatives and assist in developing and implementing best practices to improve patient outcomes.
- Adhere to infection control and safety protocols when handling and storing medications.
- Communicate effectively with patients, healthcare team members, and other stakeholders to ensure the highest level of patient care.
- Participate in interdisciplinary rounds and provide medication-related recommendations and interventions.
- Maintain inventory of medications, monitor for shortages, and assist with ordering and procurement as needed.
- Provide excellent customer service to patients and their families, addressing any concerns or questions regarding their medications.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited University.
Active Pharmacist License In The State Of Employment.
Minimum Of 2 Years Of Experience Working As A Pharmacist In An Inpatient Setting.
Knowledge Of State And Federal Regulations Related To Pharmacy Practice In An Inpatient Setting.
Strong Communication And Interpersonal Skills, With The Ability To Work Collaboratively With Other Healthcare Professionals And Patients.
