- Utilize strong nursing skills and knowledge to assess and monitor patient health status, and report any changes or concerns to the LVN Case Manager.
- Communicate effectively with patients, their families, and other healthcare professionals to provide necessary information and support.

- Assist in coordinating and scheduling appointments, procedures, and follow-up care for patients as directed by the LVN Case Manager.
- Conduct research and gather information to assist the LVN Case Manager in making informed decisions about patient care.
- Adhere to all healthcare regulations and ethical standards, maintaining confidentiality and privacy of patient information.
- Work closely with the LVN Case Manager to identify and address any barriers to patient care, providing necessary resources and support.

Valid Lvn License: The Ideal Candidate For This Role Should Hold A Valid Lvn License From An Accredited Institution. This Ensures That They Have The Necessary Education And Training To Perform Clinical Tasks And Provide Patient Care.
