Job Description

We are seeking an experienced Medical Social Worker to join our team at Kaiser Permanente. As a Medical Social Worker, you will provide mental health support and guidance to patients and families, and coordinate with other healthcare professionals to ensure the best possible outcomes. The ideal candidate will be caring, compassionate, and have a strong understanding of the medical and social needs of patients.The successful candidate will possess at least a Master’s degree in Social Work, be a licensed Social Worker in the state of practice, and have a minimum of two years of experience in a healthcare setting. The ability to communicate effectively and collaborate with other healthcare professionals is essential. Additionally, the ideal candidate will have excellent problem-solving skills, be able to work independently and with a team, and demonstrate a strong commitment to providing the highest quality of care.If you are a skilled and dedicated Medical Social Worker who is passionate about helping others, we want to hear from you!

Responsibilites:

  1. Provide mental health support and guidance to patients and families.
  2. Coordinate with other healthcare professionals to ensure the best possible outcomes.
  3. Maintain licensure and adhere to ethical standards of the social work profession.
  4. Develop and maintain effective relationships with patients, families, and other healthcare professionals.
  5. Respond to emergency and non-emergency situations in a timely and professional manner.
  6. Assess and evaluate patient needs and develop treatment plans.
  7. Provide crisis intervention, counseling, and psychosocial support to patients and families.
  8. Facilitate referrals to appropriate community resources and services.
  9. Monitor and evaluate patient progress, including the coordination of discharge planning.
  10. Document patient information and care plans according to established standards.
