
Medical Social Worker, Inpatient

- Conduct comprehensive psychosocial assessments to identify patient needs and develop individualized care plans.
- Collaborate with interdisciplinary team members to provide holistic care to patients.
- Provide counseling and support to patients and their families to address emotional, social, and financial concerns related to their medical condition.
- Advocate for patients' rights and ensure their cultural, linguistic, and religious needs are met.
- Facilitate communication and coordination between patients, families, healthcare providers, and community resources.
- Assess and address potential barriers to discharge, such as lack of insurance or housing.
- Develop and implement discharge plans to ensure a smooth transition from the hospital to the community.
- Conduct crisis intervention and risk assessments as needed.
- Document all patient interactions and interventions accurately and in a timely manner.
- Stay current with medical and social work practices and participate in continuing education opportunities.
- Maintain confidentiality of patient information in accordance with HIPAA regulations.
- Adhere to ethical standards and guidelines set by the National Association of Social Workers.
- Collaborate with community agencies and organizations to provide necessary resources and support to patients.
- Participate in team meetings, case conferences, and performance improvement activities.
- Serve as a resource and mentor to other social work staff members.
Master's Degree In Social Work From An Accredited University.
Current State License As A Licensed Clinical Social Worker (Lcsw).
Minimum Of 2 Years Of Experience In A Medical Or Healthcare Setting, Preferably In An Inpatient Setting.
Knowledge And Understanding Of Medical Terminology, Healthcare Systems, And Insurance Processes.
Strong Communication And Interpersonal Skills To Effectively Collaborate With Interdisciplinary Teams And Provide Support To Patients And Families.
