Job Description

At Kaiser Permanente, we are looking for a compassionate and dedicated Palliative Care Social Worker to join our team. In this role, you will provide psychosocial and emotional support to patients and families experiencing life-limiting illness. You will be responsible for offering counseling, providing resources, and delivering care in a manner that is sensitive, respectful, and supportive of the patient’s and family’s needs and values.The ideal candidate will have a Master’s Degree in Social Work from an accredited college or university, and have at least two years of experience in providing clinical social work services. You must have a valid state license to practice social work, and be familiar with palliative care principles and practices. You should also possess exceptional communication, problem-solving, and decision-making skills.If you are passionate about providing compassionate and supportive care to those facing life-limiting illness, we want to hear from you!

Responsibilities:

  1. Provide psychosocial and emotional support to patients and families facing life-limiting illness.
  2. Offer counseling and provide resources in a sensitive, respectful, and supportive manner.
  3. Develop and maintain a knowledge base of palliative care principles and practices.
  4. Assist with crises intervention as needed.
  5. Collaborate with multidisciplinary team to ensure comprehensive, high-quality patient care.
  6. Educate and support patients and families in their decision-making processes.
  7. Ensure adherence to professional standards and ethical codes of practice.
  8. Utilize effective communication and problem-solving skills.
  9. Maintain accurate and updated client records and document all services provided.
  10. Participate in professional development activities and continuing education programs.
