Kettering Health Network is seeking a dedicated and compassionate Social Worker to join our Care Management Acute team.   1. Provide emotional support and counseling to patients and their families during times of crisis and transition.
  2. Collaborate with multidisciplinary team members to develop and implement care plans that meet the individual needs of patients.
  3. Conduct thorough assessments of patients' social, emotional, and economic needs to identify appropriate resources and support services.
  4. Advocate for patients' rights and access to necessary resources and services.
  5. Maintain accurate and up-to-date documentation of patient interactions and progress.
  6. Facilitate communication and coordination among healthcare providers, community agencies, and other relevant parties to ensure continuity of care for patients.
  7. Educate patients and their families on available resources and support services.
  8. Participate in discharge planning and assist with the transition to post-hospital care.
  9. Collaborate with insurance companies and other financial resources to secure necessary coverage for patients.
  10. Stay current on relevant laws, regulations, and policies related to social work and healthcare.
  11. Maintain confidentiality and ethical standards in all interactions with patients and their families.
  12. Attend regular team meetings and provide updates on patient cases.
  13. Provide crisis intervention and support to patients and families as needed.
  14. Continuously evaluate and improve the effectiveness of social work services provided.
  15. Represent the organization in a positive and professional manner at all times.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work From An Accredited Institution.

  • State Licensure As A Licensed Clinical Social Worker (Lcsw) Or Licensed Independent Social Worker (Lisw).

  • Minimum Of 2 Years Of Experience In A Healthcare Setting, Preferably In Acute Care Or Hospital Social Work.

  • Knowledge And Understanding Of Government Regulations And Policies Related To Healthcare And Social Services.

  • Excellent Communication, Problem-Solving, And Decision-Making Skills, As Well As The Ability To Work Collaboratively With Interdisciplinary Teams.
