- Dispense and manage medication orders accurately and efficiently in accordance with federal and state laws, regulations, and hospital policies.
- Conduct comprehensive medication reviews and evaluations for patients to ensure safe and effective use of medications.
- Collaborate with physicians, nurses, and other healthcare professionals to provide optimal pharmaceutical care for patients.
- Monitor and assess medication therapy for effectiveness, adverse reactions, and interactions, and make appropriate recommendations.
- Educate patients and their families on medication therapy, potential side effects, and proper administration techniques.
- Participate in medication reconciliation processes to ensure accurate and complete medication histories.
- Maintain accurate and complete patient records and documentation of all pharmaceutical care activities.
- Stay current on new medications, treatment protocols, and pharmaceutical developments through continuing education and professional development.
- Comply with all safety and quality standards, including proper handling and storage of medications and adherence to infection control protocols.
- Act as a resource for other healthcare professionals by providing drug information and answering medication-related questions.
- Assist with formulary management, medication utilization review, and medication-related quality improvement initiatives.
- Adhere to all hospital policies and procedures, including those related to patient confidentiality and ethical standards.
- Serve as a preceptor for pharmacy students and residents, providing guidance and mentorship as needed.
- Participate in departmental meetings, committees, and other activities to contribute to the improvement of pharmaceutical care services.
- Maintain a positive and professional demeanor in all interactions with patients, families, and colleagues.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Pharmacy School
Active And Unrestricted Pharmacist License In The State Of Employment
Minimum Of 2 Years Of Clinical Pharmacy Experience In A Hospital Or Long-Term Care Setting
Board Certification In Pharmacotherapy (Bcps) Or Ambulatory Care (Bcacp) Is Preferred
Knowledge And Understanding Of Pharmacy Regulations, Policies, And Procedures, Including Compliance With Joint Commission And Cms Standards.
