
Community Health Worker
Legacy Health is looking for a compassionate and dedicated Community Health Worker to join our team in providing excellent healthcare services to our community. This position requires a strong understanding of and commitment to public health principles and the ability to work with diverse populations. The Community Health Worker will serve as a bridge between the healthcare team and the public, while providing support, education, and advocacy for those in need.The ideal candidate for this position is an individual who has a passion for giving back to the community, has excellent interpersonal and communication skills and is committed to helping others. A minimum of a high school diploma is required along with a minimum of two years’ experience in a related field. Previous experience in the healthcare industry, social services, or public health is preferred. The successful candidate must be comfortable working with diverse populations and have an understanding of the challenges faced by underserved communities. Responsibilities:
- Provide support, education and advocacy to members of underserved communities.
- Establish and maintain strong relationships with healthcare providers, community members and other stakeholders.
- Connect individuals and families with resources and services.
- Educate community members on healthcare policies, procedures, and options.
- Facilitate communication between healthcare providers, patients, and families.
- Monitor and evaluate community health initiatives and programs.
- Assist in the development and implementation of community health initiatives.
- Maintain accurate and up to date records of health care services provided.
- Participate in health fairs, community outreach events, and other activities.
- Develop and implement patient education materials.
