1. Establish and maintain strong relationships with hospital staff, including physicians, nurses, and discharge planners.
2. Coordinate with hospital staff to identify potential home care patients and facilitate the discharge process.
3. Educate hospital staff on our company's services and capabilities to ensure a seamless transition for patients.
4. Conduct thorough assessments of patients' needs and communicate this information to our team to ensure appropriate care is provided.
5. Act as a resource for hospital staff, providing guidance and support for any questions or concerns related to home care.
6. Collaborate with our team to develop individualized care plans for patients and ensure their needs are being met.
7. Monitor and track patient progress and communicate any changes or concerns to hospital staff and appropriate team members.
8. Maintain knowledge of current healthcare regulations and ensure compliance with all applicable laws and guidelines.
9. Participate in marketing and community outreach efforts to promote our services and build relationships with potential referral sources.
10. Continuously seek out opportunities to improve processes and enhance the patient experience.
