- Provide safe and effective medication therapy management to patients by reviewing and verifying prescriptions, monitoring drug interactions, and counseling patients on proper medication usage.
- Collaborate with healthcare professionals, including physicians, nurses, and other pharmacists, to ensure appropriate medication therapy for patients.
- Stay updated on current pharmaceutical research, trends, and developments to provide the most advanced and effective care to patients.
- Maintain accurate and complete patient records, including medication histories, allergies, and drug interactions.
- Participate in multidisciplinary rounds and provide drug information to healthcare team members to optimize patient care.
- Assist in the development and implementation of medication protocols and guidelines to ensure safe and efficient medication use.
- Educate and counsel patients on their medications, including proper administration, potential side effects, and drug interactions.
- Monitor and manage medication inventory and ensure proper storage and handling of medications.
- Adhere to all federal and state laws and regulations, as well as hospital policies and procedures, related to medication use and patient safety.
- Serve as a resource for patients, families, and healthcare providers regarding medication-related questions and concerns.
- Continuously assess and improve medication therapy processes to enhance patient outcomes and quality of care.
- Participate in professional development activities to maintain and enhance knowledge and skills in the field of pharmacy.
- Uphold the mission and values of Loma Linda University and promote a culture of excellence, compassion, and patient-centered care.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Pharmacy School.
Current And Valid State License To Practice Pharmacy.
Completion Of A Postgraduate Residency Program In Clinical Pharmacy.
Board Certification In A Relevant Area Of Specialization (E.g. Ambulatory Care, Critical Care, Oncology).
Experience In A Hospital Or Healthcare Setting, Preferably In A Clinical Pharmacy Role.
