  1. Conduct medication therapy management for patients to ensure safe and effective use of medications.
  2. Collaborate with healthcare team members to develop and implement treatment plans for patients.
  3. Review medication orders and patient medical histories to identify potential drug interactions and contraindications.
  4. Monitor patient response to medication therapy and make recommendations for adjustments as needed.
  5. Provide patient education on proper medication use, potential side effects, and other relevant information.
  6. Maintain accurate and complete patient medication records.
  7. Participate in medication reconciliation processes to ensure seamless transitions of care.
  8. Stay updated on new medications and therapies, and provide recommendations for their use in patient care.
  9. Work closely with physicians and other healthcare providers to promote interdisciplinary collaboration and improve patient outcomes.
  10. Adhere to all state and federal laws and regulations related to medication dispensing and administration.
  11. Serve as a resource for pharmacy technicians and other support staff, providing guidance and support as needed.
  12. Participate in quality improvement initiatives to enhance patient care and safety.
  13. Conduct drug utilization reviews to identify opportunities for cost savings and improved patient outcomes.
  14. Maintain a professional and ethical demeanor at all times, representing the organization in a positive manner.
  15. Continuously strive to improve patient care and medication management processes through ongoing education and self-development.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution: A Clinical Pharmacist At Loma Linda University Must Possess A Pharmd Degree From An Accredited Institution, Demonstrating A Strong Foundation In Pharmaceutical Sciences And Clinical Practice.

  • Active Pharmacy License In The State Of California: The Candidate Must Hold A Current And Valid Pharmacy License In The State Of California, Allowing Them To Practice As A Pharmacist And Dispense Medications Within The State.

  • Board Certification In Pharmacotherapy Or Ambulatory Care: Loma Linda University Values Board-Certified Pharmacists And Prefers Candidates Who Have Obtained Board Certification In Pharmacotherapy Or Ambulatory Care. This Demonstrates A High Level Of Expertise And Commitment To The Field Of Clinical Pharmacy.

  • Residency Or Fellowship Training In Clinical Pharmacy: A Residency Or Fellowship In Clinical Pharmacy Is Highly Desired For This Position. This Additional Training Provides Hands-On Experience In Various Clinical Settings And Allows The Candidate To Develop Advanced Skills In Patient Care And Medication Management.

  • Excellent Communication And Interpersonal Skills: As A Clinical Pharmacist, The Candidate Will Be Working Closely With Patients, Healthcare Professionals, And Other Members Of The Healthcare Team. Strong Communication And Interpersonal Skills Are Crucial For Effectively Collaborating And Providing Patient-Centered Care.
