Job Description

We are seeking a highly skilled and compassionate Clinical Pharmacist to join our team at Mainehealth, specifically in the critical care department. In this role, you will play a crucial part in ensuring the safe and effective use of medications for our critically ill patients.   1. Dispense medications to critically ill patients in a timely and accurate manner.
  2. Review patient medical histories and assess medication orders for appropriateness and potential drug interactions.
  3. Collaborate with physicians and other healthcare providers to develop optimal medication treatment plans for critically ill patients.
  4. Monitor patient response to medications and make necessary adjustments to ensure safety and effectiveness.
  5. Educate patients and their families on proper medication use and potential side effects.
  6. Participate in drug utilization reviews and medication error reduction initiatives.
  7. Maintain accurate and complete patient medication records.
  8. Stay up-to-date on the latest advancements and guidelines in critical care pharmacy.
  9. Provide guidance and support to pharmacy staff and other healthcare professionals.
  10. Adhere to all state and federal laws and regulations related to pharmacy practice.
  11. Act as a resource for medication-related inquiries from healthcare providers, patients, and their families.
  12. Participate in multidisciplinary rounds and provide input on medication therapy for critically ill patients.
  13. Ensure medication stock levels are adequate and properly stored.
  14. Continuously evaluate and improve pharmacy processes and procedures.
  15. Maintain a high level of professionalism and confidentiality in all interactions with patients and colleagues.
  16. Attend and participate in departmental meetings and trainings as required.
  17. Adhere to all safety protocols and procedures to maintain a safe working environment for both staff and patients.
  18. Collaborate with pharmacy leadership to develop and implement quality improvement initiatives.
  19. Act as a role model for other pharmacists and pharmacy staff by demonstrating excellent clinical skills and compassionate patient care.
  20. Maintain a positive attitude and contribute to a collaborative and supportive work environment.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited College Of Pharmacy

  • Completion Of A Pgy1 Residency Program In Pharmacy Practice Or A Pgy2 Residency Program In Critical Care Pharmacy

  • Board Certification In Pharmacotherapy (Bcps) Or Critical Care Pharmacy (Bcccp)

  • Current Licensure As A Registered Pharmacist In The State Of Maine

  • Previous Experience In A Critical Care Setting, Preferably In An Acute Care Hospital Or Healthcare System.
