  1. Provide high-quality care to hospice patients and their families.
  2. Serve as a liaison between patients, families, and healthcare team members.
  3. Coordinate and manage the care plan for hospice patients.
  4. Assess patient needs and develop individualized care plans.
  5. Collaborate with other healthcare professionals to ensure comprehensive care.
  6. Administer medications and treatments according to physician orders.
  7. Educate patients and families on end-of-life care options and resources.
  8. Monitor patient progress and make necessary adjustments to care plans.
  9. Ensure all documentation and records are accurately maintained.
  10. Communicate effectively with patients, families, and healthcare team members.
  11. Provide emotional support and counseling to patients and families.
  12. Participate in interdisciplinary team meetings and provide input on patient care.
  13. Stay current with hospice care regulations and best practices.
  14. Maintain a safe and clean environment for patients.
  15. Continuously evaluate and improve care processes to ensure high-quality care.
  16. Uphold Mayo Clinic's values of compassion, excellence, respect, and teamwork.
  17. Participate in ongoing education and training opportunities.
  18. Maintain confidentiality of patient information.
  19. Adhere to all policies and procedures of Mayo Clinic.

Job Qualifications
  • Bachelor's Degree In Nursing From An Accredited Institution

  • Current Rn License In The State Of Employment

  • Minimum Of 2 Years Of Hospice Nursing Experience

  • Experience In Care Coordination And Case Management

  • Knowledge Of Hospice Regulations And Documentation Requirements
