- Provide high-quality pharmaceutical care to patients with infectious diseases and those receiving outpatient parenteral antibiotic therapy (OPAT).
- Collaborate with interdisciplinary team members to develop and implement treatment plans for patients with complex medication needs.
- Utilize clinical knowledge and expertise to optimize medication therapy and ensure positive patient outcomes.
- Monitor drug therapy regimens and make recommendations for adjustments as needed.
- Stay up-to-date on current research, guidelines, and best practices related to infectious diseases and OPAT.
- Conduct medication reconciliation and review for accuracy and appropriateness.
- Educate patients and their caregivers on proper medication usage and potential side effects.
- Communicate effectively with healthcare providers, patients, and other team members to ensure coordinated and comprehensive care.
- Participate in quality improvement initiatives and other projects related to infectious diseases and OPAT.
- Maintain accurate and complete patient records in compliance with regulatory standards.
- Continuously expand knowledge and skills through professional development opportunities.
- Adhere to all pharmacy laws, regulations, and policies to ensure patient safety and confidentiality.
- Serve as a resource and mentor to other pharmacy staff members.
- Promote a culture of patient-centered care and excellence in pharmacy practice.
- Represent Mayo Clinic by demonstrating professionalism, compassion, and respect towards patients, colleagues, and the community.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited College Of Pharmacy.
Completion Of A Residency In Infectious Diseases Or A Related Field (E.g. Internal Medicine, Critical Care) Preferred.
Board Certification In Infectious Diseases (Bcid) Or Pharmacotherapy (Bcps) Preferred.
Experience In Managing Patients On Outpatient Parenteral Antimicrobial Therapy (Opat) Preferred.
Strong Knowledge Of Infectious Diseases, Antimicrobial Stewardship Principles, And Drug Interactions.
