
Solid Organ Transplant Pharmacist
The Mayo Clinic is seeking an experienced and professional Solid Organ Transplant Pharmacist to join their team. The ideal candidate will have a passion for patient care and a commitment to delivering high quality pharmaceutical care to our transplant patients. The Solid Organ Transplant Pharmacist will be responsible for the safe and effective management of the medication therapy of transplant patients.The successful candidate will have a Doctor of Pharmacy degree from an accredited college of pharmacy, be licensed to practice in the state of Minnesota, and have at least two years of experience in an acute care setting. Preference will be given to those with experience in solid organ transplant or critical care. The Solid Organ Transplant Pharmacist must possess excellent communication and interpersonal skills, and be able to work effectively in a fast-paced, ever-changing environment.
Responsibilities: • Manage medication therapy of transplant patients in a safe and effective manner • Understand and apply medication therapies for organ transplants • Participate in patient-specific rounds, consults, and discharge planning • Develop and implement patient-specific therapeutic plans • Ensure compliance with regulations and policies regarding medication therapy • Monitor patient progress and adjust therapy as needed • Educate patients, families, and healthcare providers on the management of transplant related medication therapy • Collaborate with team members to ensure optimal patient outcomes • Participate in the development and implementation of quality assurance and performance improvement initiatives.
