  1. Provide compassionate care and support to patients and their families in accordance with MedStar Health's mission and values.
  2. Serve as a liaison between patients, families, and healthcare providers to ensure effective communication and coordination of care.
  3. Assess the social, emotional, and psychological needs of patients and develop individualized care plans.
  4. Collaborate with interdisciplinary teams to develop and implement treatment plans that address patients' physical, emotional, and social needs.
  5. Advocate for patients' rights and provide guidance and support in navigating healthcare systems and accessing resources.
  6. Conduct psychosocial assessments and provide crisis intervention and counseling services as needed.
  7. Monitor and evaluate patients' progress and make appropriate referrals to community resources and support services.
  8. Maintain accurate and timely documentation of patient interactions and interventions.
  9. Stay updated on current social work practices, policies, and regulations and adhere to ethical standards in all interactions.
  10. Participate in team meetings, case conferences, and other collaborative efforts to improve patient care and outcomes.
  11. Collaborate with community organizations and agencies to develop partnerships and resources for patients.
  12. Provide education and support to patients and their families on coping strategies, disease management, and self-care.
  13. Identify and address any barriers to care and assist patients in overcoming them.
  14. Participate in quality improvement activities and contribute to the development of policies and procedures.
  15. Attend trainings, workshops, and conferences to enhance knowledge and skills in social work and healthcare.

Job Qualifications
  • Master's Degree In Social Work From An Accredited University, With A Focus On Healthcare Or Gerontology.

  • Minimum Of 3 Years Experience In A Healthcare Setting, Preferably In A Care Management Or Discharge Planning Role.

  • Current State License To Practice Social Work, With Lcsw Preferred.

  • Strong Understanding Of Healthcare Systems, Insurance Plans, And Medicare/Medicaid Regulations.

  • Excellent Communication And Interpersonal Skills, With The Ability To Work Collaboratively With A Multidisciplinary Team And Advocate For Patients' Needs.
