  1. Assist the Case Management department in providing comprehensive care coordination services to patients.
  2. Welcome and greet patients and their families with warmth and compassion.
  3. Maintain a high level of organization and efficiency in managing patient information and records.
  4. Collaborate with the Case Management team to create and implement individualized care plans for patients.
  5. Communicate effectively with patients, healthcare providers, and insurance companies to ensure seamless delivery of care.
  6. Assist in identifying and connecting patients with community resources and support programs.
  7. Coordinate appointments, referrals, and other necessary services for patients.
  8. Maintain confidentiality and adhere to all HIPAA regulations and guidelines.
  9. Support the Case Management team in advocating for patients and ensuring their needs are met.
  10. Document all interactions and updates in the patient's electronic medical record.
  11. Attend meetings and training sessions to stay updated on best practices and procedures.
  12. Continuously strive to improve the patient experience and provide exceptional customer service.
  13. Adhere to all MedStar Health policies and procedures.
  14. Work collaboratively with colleagues and contribute to a positive and supportive team environment.

Job Qualifications
