  1. Conduct medication reviews and assessments for patients to ensure safe and effective use of medications.
  2. Collaborate with healthcare team members to develop individualized treatment plans for patients.
  3. Monitor and adjust medication regimens based on patient response and laboratory data.
  4. Provide drug information and education to patients, caregivers, and healthcare professionals.
  5. Ensure compliance with all state and federal regulations related to medication management.
  6. Participate in medication reconciliation processes to reduce medication errors.
  7. Stay up-to-date with current research and guidelines related to medication therapy.
  8. Work closely with physicians, nurses, and other healthcare professionals to optimize patient outcomes.
  9. Assess medication-related problems and make recommendations for improvement.
  10. Participate in multidisciplinary rounds and provide input on medication-related issues.
  11. Serve as a resource for medication-related questions and concerns from patients and healthcare team members.
  12. Maintain accurate and complete documentation of all medication-related activities.
  13. Educate patients on proper medication use, potential side effects, and drug interactions.
  14. Oversee the storage, preparation, and dispensing of medications.
  15. Continuously seek opportunities to improve medication management processes and patient outcomes.
  16. Act as a role model for ethical and professional behavior in all interactions.
  17. Participate in quality improvement initiatives and assist in developing and implementing medication-related policies and procedures.
  18. Maintain professional competence through participation in continuing education programs and professional organizations.
  19. Actively contribute to a positive and collaborative work environment.
  20. Adhere to all safety and infection control policies and procedures.

Job Qualifications
  • Doctor Of Pharmacy (Pharm.d.) Degree From An Accredited Pharmacy School.

  • Active And Unrestricted Pharmacy License In The State Of Employment.

  • Board Certification In Pharmacotherapy Or Other Relevant Specialty.

  • Minimum Of 2 Years Of Clinical Pharmacy Experience In A Hospital Or Healthcare Setting.

  • Strong Communication And Collaboration Skills, With The Ability To Work Effectively In A Multidisciplinary Team.
