
Diabetes Educator - Registered Nurse

- Develop and implement diabetes education programs for patients of all ages and backgrounds.
- Conduct individual and group education sessions to teach patients about diabetes management, including medication management, blood glucose monitoring, healthy eating, and exercise.
- Assess patients' understanding of their condition and provide personalized education based on their individual needs.
- Collaborate with healthcare team members, including physicians and nurses, to develop individualized care plans for patients with diabetes.
- Stay up-to-date on the latest research and developments in diabetes care and share this information with patients and colleagues.
- Serve as a resource for patients, families, and healthcare providers regarding diabetes education, management, and prevention.
- Provide emotional support and encouragement to patients as they navigate the challenges of living with diabetes.
- Monitor patients' progress and adjust education plans as needed to ensure optimal outcomes.
- Document patient education and progress in medical records.
- Participate in community outreach programs to raise awareness about diabetes and promote healthy lifestyle choices.
- Adhere to all HIPAA regulations and maintain patient confidentiality at all times.
- Continually seek opportunities for professional development and attend relevant workshops, conferences, and training sessions.
- Maintain a positive and professional attitude, creating a safe and welcoming environment for patients.
- Uphold the values and mission of the organization while representing it to patients, families, and the community.
Current Rn Licensure: A Diabetes Educator-Registered Nurse At Mercy Must Hold A Current And Valid Rn License In The State Where They Will Be Practicing.
Certified Diabetes Educator (Cde) Certification: Candidates Must Possess A Cde Certification From The National Certification Board For Diabetes Educators (Ncbde) To Be Considered For The Role.
Bachelor Of Science In Nursing (Bsn): A Bsn Degree Is Required For This Position, As It Provides A Strong Foundation In Nursing Theory, Research, And Leadership Skills.
Minimum Of 2 Years Of Experience In Diabetes Management: Candidates Must Have At Least 2 Years Of Experience Working In A Clinical Setting, With A Focus On Diabetes Management And Education.
Strong Communication And Teaching Skills: Diabetes Educators Must Have Excellent Communication Skills And Be Able To Effectively Educate Patients And Their Families On Diabetes Self-Management Techniques, Treatment Plans, And Lifestyle Modifications.
