4. Collaborate with healthcare team members, including physicians and nurses, to develop individualized care plans for patients with diabetes.

6. Serve as a resource for patients, families, and healthcare providers regarding diabetes education, management, and prevention.
7. Provide emotional support and encouragement to patients as they navigate the challenges of living with diabetes.
8. Monitor patients' progress and adjust education plans as needed to ensure optimal outcomes.
9. Document patient education and progress in medical records.
10. Participate in community outreach programs to raise awareness about diabetes and promote healthy lifestyle choices.
11. Adhere to all HIPAA regulations and maintain patient confidentiality at all times.
12. Continually seek opportunities for professional development and attend relevant workshops, conferences, and training sessions.
13. Maintain a positive and professional attitude, creating a safe and welcoming environment for patients.
14. Uphold the values and mission of the organization while representing it to patients, families, and the community.
