- Provide emotional, spiritual, and practical support to individuals and families who have experienced the loss of a loved one.
- Help individuals and families navigate the difficult journey of grief and find healthy coping mechanisms.
- Conduct assessments to determine the needs of bereaved individuals and develop appropriate care plans.
- Collaborate with other members of the hospice team to coordinate and provide comprehensive care.
- Offer group and individual counseling sessions to support individuals and families in their grieving process.
- Educate individuals and families on the grieving process and provide resources for additional support.
- Maintain accurate and thorough documentation of interactions with bereaved individuals and families.
- Advocate for the needs of bereaved individuals and families within the healthcare system.
- Collaborate with community organizations and resources to provide additional support to bereaved individuals and families.
- Stay up-to-date on best practices and interventions for supporting individuals and families during times of loss and grief.
- Provide support and guidance to hospice team members in supporting bereaved individuals and families.
- Maintain confidentiality and ethical standards in all interactions with individuals and families.
- Participate in team meetings and supervision to discuss and improve the quality of care provided.
- Continuously evaluate and adjust care plans to meet the changing needs of bereaved individuals and families.
- Demonstrate empathy, patience, and compassion in all interactions with bereaved individuals and families.
- Adhere to all policies and procedures of the hospice program and healthcare organization.
- Collaborate with insurance companies and other third-party payers to ensure coverage for bereavement services.
- Participate in professional development and continuing education opportunities to enhance skills and knowledge.
- Foster a compassionate and supportive work environment for team members and promote a culture of teamwork and collaboration.
- Represent the hospice program and healthcare organization in a professional and positive manner at all times.
Master's Degree In Social Work From An Accredited Program.
Current State Licensure As A Licensed Clinical Social Worker (Lcsw).
Minimum Of 2 Years Of Experience In Hospice Or Palliative Care.
Knowledge And Experience In Grief And Bereavement Counseling.
Ability To Work Collaboratively As Part Of An Interdisciplinary Team And Provide Support To Patients And Families During End-Of-Life Care.
