Your expertise in providing emotional and practical support will be essential in helping our patients and their loved ones navigate this challenging time.To excel in this role, you must have a strong understanding of the physical, emotional, and spiritual needs of hospice patients and their families. Additionally, a Master's degree in Social Work, along with relevant experience in a healthcare setting, is required.   1. Provide emotional and practical support to hospice patients and their families during their end-of-life journey.
  2. Utilize expertise in social work to assess and address the physical, emotional, and spiritual needs of hospice patients and their families.
  3. Collaborate with interdisciplinary team to develop and implement individualized care plans for patients.
  4. Conduct thorough assessments of patients' psychosocial needs and develop appropriate interventions.
  5. Offer counseling and support services to patients and their families, including bereavement support.
  6. Advocate for patients' rights and ensure their needs are met by coordinating with other healthcare professionals and community resources.
  7. Maintain accurate and timely documentation of patient interactions and progress.
  8. Provide education to patients and families on hospice care and end-of-life planning.
  9. Adhere to ethical and legal standards in all aspects of patient care.
  10. Participate in team meetings and case conferences to discuss patient care and contribute to care planning.
  11. Collaborate with community organizations and resources to provide additional support to patients and families.
  12. Stay updated on current social work practices, policies, and regulations related to hospice care.
  13. Maintain confidentiality and respect for patients and their families at all times.
  14. Demonstrate compassion, empathy, and sensitivity in all interactions with patients and families.
  15. Foster a positive and supportive environment for patients, families, and colleagues.

Job Qualifications
  • Current Licensure As A Licensed Clinical Social Worker (Lcsw) In The State Of Employment.

  • Bachelor's Or Master's Degree In Social Work From An Accredited University.

  • Minimum Of 2 Years Of Experience In A Hospice Or Palliative Care Setting.

  • Strong Understanding Of End-Of-Life Care, Grief And Bereavement, And Ethical Issues Related To Hospice Social Work.

  • Excellent Communication And Interpersonal Skills, With The Ability To Provide Emotional Support And Counseling To Patients And Their Families.
