
Clinical Pharmacist
Meritus Health is seeking an experienced and passionate Clinical Pharmacist to join our team. In this role, you will be responsible for providing comprehensive pharmaceutical care and therapeutic drug monitoring services to our patients. You will work collaboratively with our multidisciplinary healthcare team to ensure the highest quality of patient care and satisfaction. The ideal candidate has a Doctor of Pharmacy degree (Pharm.D.) from an accredited program, as well as a valid license to practice pharmacy in the state of Maryland. They possess excellent interpersonal and organizational skills, and display strong problem-solving capabilities. They must be detail-oriented and have a passion for providing the highest quality of care and service to our patients. Additionally, the successful candidate will demonstrate the ability to work independently and collaboratively in a fast-paced, demanding environment. Responsibilities:
- Provide comprehensive pharmaceutical care and therapeutic drug monitoring services to patients.
- Work collaboratively with multidisciplinary healthcare team to ensure highest quality of patient care and satisfaction.
- Ensure compliance with all applicable state and federal regulations.
- Develop and maintain relationships with patients, medical staff, and other members of the healthcare team.
- Participate in professional development activities to stay up-to-date on new advances in the pharmacy field.
- Utilize clinical knowledge and decision-making skills to provide safe and effective medication therapy to patients.
- Monitor patient progress and outcomes to ensure patient safety and satisfaction.
- Document patient medication records accurately and in accordance with departmental policies and procedures.
- Manage drug inventories and ensure the accuracy of medication orders.
- Provide patient education on the proper use of medications and potential side effects.
