Job Description

Moffitt Cancer Center is looking for an experienced and passionate Case Manager to join our team. As a Case Manager you will provide comprehensive care coordination and case management services to our patients, families, and caregivers. You will be an integral member of the interdisciplinary team, providing support and guidance in navigating the healthcare system and coordinating services for our patients. The ideal candidate has a minimum of two years of experience in a healthcare setting, preferably in case management, and a commitment to compassionate, patient-centered care. Responsibilities:

  • Provide comprehensive care coordination and case management services to patients, families, and caregivers.
  • Collaborate with interdisciplinary team to coordinate services for patients and navigate the healthcare system.
  • Develop and implement care plans for patients in accordance with established standards and protocols.
  • Monitor patients’ progress and adjust plans accordingly.
  • Assist patients in accessing necessary resources and support services.
  • Facilitate communication between patients, families, caregivers, and healthcare team.
  • Document patient encounters and care plans in medical records.
  • Stay current on best practices and emerging trends in case management.
  • Remain compliant with all applicable laws and regulations.
  • Participate in professional development activities and continuing education opportunities.
