
Mental Health Worker
At Northwestern Medicine, we are looking for an enthusiastic and dedicated Mental Health Worker to provide compassionate and comprehensive care to our patients. Our ideal candidate will have excellent communication skills, a compassionate and caring attitude, and the ability to work collaboratively with our multidisciplinary team.The Mental Health Worker will provide support to patients and their families by providing crisis management, mental health assessments, and psychosocial interventions. As a member of the team, you will be responsible for identifying and assessing patient needs, maintaining patient records, and developing and implementing individualized treatment plans.The successful candidate will possess a Bachelor’s degree in Human Services, Social Work, Psychology, or a related field. Experience working in a mental health setting is preferred. Responsibilities:
- Provide compassionate and comprehensive care to patients and their families, including crisis management, mental health assessments, and psychosocial interventions.
- Identify and assess patient needs, maintain patient records, and develop and implement individualized treatment plans.
- Provide support to the multidisciplinary team in order to ensure the best possible patient care.
- Demonstrate a strong commitment to providing patient-centered care and an understanding of how to meet the needs of individuals in a variety of contexts.
- Maintain confidentiality and professional boundaries with patients and their families.
- Adhere to all applicable laws, regulations, and policies.
- Participate in continuing education and training programs as required.
