
Advanced Practice Provider - Palliative Care

In this role, you will work alongside a multidisciplinary team to provide specialized care to patients with serious or chronic illnesses, focusing on symptom management, quality of life, and patient-centered support. - Provide specialized clinical care to patients with serious or chronic illnesses, focusing on symptom management, quality of life, and patient-centered support.
- Collaborate with a multidisciplinary team to develop comprehensive care plans for patients.
- Conduct thorough assessments, including physical and psychosocial evaluations, to determine appropriate treatment plans.
- Administer medications, treatments, and procedures as prescribed by physicians.
- Monitor and evaluate patient progress, adjusting care plans as needed.
- Educate patients and their families on disease processes, treatment options, and self-care management strategies.
- Advocate for patients and families to ensure their needs and wishes are addressed and respected.
- Document all patient care activities accurately and timely.
- Stay up-to-date on current evidence-based practices and participate in ongoing education and training.
- Adhere to all policies, procedures, and regulations of Orlando Health.
- Maintain confidentiality and professionalism at all times.
- Communicate effectively with patients, families, and colleagues to promote a positive and collaborative work environment.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Serve as a resource and mentor to other healthcare team members.
- Maintain a positive attitude and promote a culture of compassion, respect, and excellence within the Palliative Care team.
- Perform other duties and responsibilities as assigned by the supervisor.
Master's Degree In Nursing Or Related Field: An Advanced Practice Provider In Palliative Care At Orlando Health Must Possess A Master's Degree In Nursing Or A Related Field To Demonstrate A High Level Of Knowledge And Expertise In This Specialized Area Of Healthcare.
Board Certification: Candidates For This Position Should Be Board Certified As A Nurse Practitioner (Np) Or Physician Assistant (Pa) In Palliative Care. This Certification Ensures That The Individual Has Met The Rigorous Standards And Competencies Set By A National Certifying Body.
Minimum 2 Years Of Experience In Palliative Care: Orlando Health Requires A Minimum Of 2 Years Of Experience Working In Palliative Care For This Position. This Ensures That The Candidate Has A Strong Foundation In Providing Comprehensive Care To Patients With Serious, Life-Limiting Illnesses.
Excellent Communication And Interpersonal Skills: Effective And Compassionate Communication Is Crucial In Palliative Care, As Providers Must Navigate Difficult Conversations With Patients And Families. Therefore, Candidates For This Position Should Possess Excellent Communication And Interpersonal Skills To Effectively Collaborate With Interdisciplinary Teams And Provide Emotional Support To Patients And Families.
Knowledge Of End-Of-Life Care And Pain Management: Palliative Care Providers Must Have A Deep Understanding Of End-Of-Life Care And Pain Management. Candidates For This Position Should Have A Strong Knowledge Of Evidence-Based Practices And Guidelines For Managing Pain And Other Symptoms Associated With Serious Illnesses. They Should Also Possess A Compassionate And Empathetic Approach To Supporting Patients And Families During This Difficult Time.
