  1. Conduct comprehensive assessments of patients to determine their care needs and create individualized care plans.
  2. Collaborate with healthcare team members to coordinate and facilitate patient care, ensuring seamless transitions between different levels of care.
  3. Advocate for patients and their families, ensuring their needs and preferences are considered in the care planning process.
  4. Provide emotional support and counseling to patients and families, addressing any psychosocial concerns that may impact their health and well-being.
  5. Maintain accurate and up-to-date documentation of patient interactions and progress in electronic medical records.
  6. Educate patients and families about healthcare resources, support services, and community resources available to them.
  7. Identify and address any barriers to care, such as financial, cultural, or logistical, and work to find solutions.
  8. Collaborate with community organizations and agencies to ensure patients have access to necessary resources and support systems.
  9. Keep up-to-date on changes and advancements in the healthcare industry and apply this knowledge to improve patient care.
  10. Adhere to all ethical and legal standards in the provision of care and maintain patient confidentiality.
  11. Communicate effectively with patients, families, and healthcare team members to ensure a positive and collaborative care experience.
  12. Participate in quality improvement initiatives to enhance the delivery of care and outcomes for patients.
  13. Serve as a resource for other healthcare team members, providing guidance and support on social work-related issues.

Job Qualifications
  • Master's Degree In Social Work Or Related Field From An Accredited University.

  • Current Licensure As A Clinical Social Worker In The State Of Florida.

  • Minimum Of 3 Years Of Experience In A Healthcare Setting, Preferably In A Care Coordination Or Case Management Role.

  • Strong Knowledge Of Medical Terminology, Insurance Regulations, And Government Programs (Medicaid, Medicare, Etc.).

  • Excellent Communication And Interpersonal Skills, With The Ability To Work Collaboratively With Interdisciplinary Teams And Effectively Advocate For Patients.
