  1. Conduct patient assessments and develop individualized treatment plans to improve patient outcomes and quality of care.
  2. Collaborate with physicians and other healthcare professionals to provide comprehensive pharmaceutical care to patients.
  3. Monitor and review medication therapy to ensure safety, effectiveness, and appropriateness for each patient.
  4. Stay up-to-date on the latest developments and advancements in clinical pharmacy practice, and integrate this knowledge into daily practice.
  5. Serve as a resource for other healthcare professionals regarding medication therapy, drug interactions, and pharmaceutical interventions.
  6. Participate in multidisciplinary rounds and meetings to discuss patient care and make recommendations for therapy adjustments.
  7. Maintain accurate and thorough documentation of patient care activities.
  8. Educate patients and their families on medication therapy, potential side effects, and adherence strategies.
  9. Contribute to quality improvement initiatives and assist with developing and implementing evidence-based practices within the organization.
  10. Maintain compliance with all state and federal laws, regulations, and standards related to pharmacy practice.
  11. Act as a mentor and preceptor for pharmacy students and residents, providing guidance and support in their learning and development.
  12. Collaborate with the pharmacy team to ensure adequate medication supply and inventory management.
  13. Continuously assess and identify opportunities for process improvement and implement changes to enhance patient care and workflow efficiency.
  14. Participate in departmental and organizational committees and initiatives to promote best practices and quality patient care.
  15. Exemplify a strong commitment to patient safety, confidentiality, and ethical standards in all job duties.

Job Qualifications
  • Doctor Of Pharmacy (Pharm.d.) Degree From An Accredited Pharmacy School.

  • Completion Of A Post-Graduate Residency Program In Clinical Pharmacy Or Specialized Area.

  • Board Certification In A Relevant Area Of Clinical Pharmacy (E.g. Pharmacotherapy, Ambulatory Care, Critical Care).

  • Current And Unrestricted Pharmacist License In The State Of Florida.

  • Minimum Of 3-5 Years Of Experience Practicing As A Clinical Pharmacist In A Hospital Or Healthcare Setting.
