Job Qualifications
  • Bachelor's Degree In Nursing, Public Health, Or Related Field.

  • Minimum Of 3 Years Experience In Population Health Management Or Care Coordination.

  • Strong Understanding Of Healthcare Regulations And Policies, Including Hipaa And Medicaid/Medicare.

  • Excellent Communication And Interpersonal Skills To Effectively Collaborate With Patients, Healthcare Providers, And Community Organizations.

  • Proficient In Data Analysis And Technology, With The Ability To Use Electronic Health Records And Population Health Management Software.
