Bachelor's Degree In Nursing, Public Health, Or Related Field.
Minimum Of 3 Years Experience In Population Health Management Or Care Coordination.
Strong Understanding Of Healthcare Regulations And Policies, Including Hipaa And Medicaid/Medicare.
Excellent Communication And Interpersonal Skills To Effectively Collaborate With Patients, Healthcare Providers, And Community Organizations.
Proficient In Data Analysis And Technology, With The Ability To Use Electronic Health Records And Population Health Management Software.
