- Provide direct patient care to geriatric population, including conducting physical exams, diagnosing and treating illnesses, and developing individualized care plans.
- Collaborate with interdisciplinary team to ensure comprehensive and coordinated care for older adults.
- Utilize evidence-based practices and current research to provide high-quality, patient-centered care.
- Educate patients and their families about healthcare needs, treatment plans, and self-management strategies.
- Conduct thorough assessments and screenings to identify any physical, emotional, or social concerns and address them appropriately.
- Monitor and manage chronic health conditions in older adults, including medication management and follow-up care.
- Maintain accurate and timely documentation of patient care, including progress notes, treatment plans, and other necessary records.
- Participate in quality improvement initiatives to enhance the overall healthcare experience for older adults.
- Serve as a resource and mentor for other healthcare professionals, including nursing staff and students.
- Stay current with developments in geriatric medicine and maintain necessary certifications and licensure.
- Demonstrate empathy, compassion, and sensitivity when working with older adults and their families.
- Adhere to all ethical and legal standards of practice.
- Communicate effectively and collaborate with patients, families, and colleagues to ensure positive outcomes.
- Maintain a safe and therapeutic environment for patients and staff.
- Uphold the mission, vision, and values of the healthcare organization.
Master's Degree In Adult-Gerontology Nurse Practitioner (Agnp) From An Accredited Program.
Minimum Of 2 Years Of Clinical Experience As An Agnp, Preferably In A Primary Or Acute Care Setting.
Board Certification As An Agnp Through The American Nurses Credentialing Center (Ancc) Or The American Association Of Nurse Practitioners (Aanp).
Current And Unrestricted State Licensure As An Advanced Practice Registered Nurse (Aprn) In The State Of New Mexico.
Strong Clinical Skills And Knowledge In Managing Complex Adult And Geriatric Conditions, Including Chronic Diseases And Acute Illnesses.
