5. Advocate for patients and families in the healthcare system.
6. Build relationships with community resources to support patient and family needs.
7. Offer emotional support and guidance to patients and families during difficult times.
8. Monitor patient progress and adjust care plans accordingly.
9. Facilitate patient and family meetings to discuss end-of-life care.
10. Remain up-to-date on healthcare regulations, best practices, and new treatments.
