Outpatient Social Worker Palliative Care/Hospice

The successful candidate should have a passion for helping others and a commitment to providing quality care with empathy and understanding.The Outpatient Social Worker Palliative Care/Hospice will be responsible for assessing the needs of patients, communicating with the interdisciplinary care team, and developing plans to help patients and their families cope with the physical and emotional aspects of their illness. The ideal candidate will possess excellent communication skills, a strong work ethic, and the ability to maintain a professional and empathetic demeanor.To be considered for this role, successful applicants will need a Master’s Degree in Social Work with a minimum of two (2) years of experience working in a palliative care and/or hospice setting. A valid Social Worker license in the State of New Mexico is also required.
