
Outpatient Social Worker Palliative Care/Hospice
At Presbyterian Healthcare Services, we are looking for a compassionate and dedicated Outpatient Social Worker Palliative Care/Hospice to provide support to patients and their families who are dealing with serious illness or end of life care. The successful candidate should have a passion for helping others and a commitment to providing quality care with empathy and understanding.The Outpatient Social Worker Palliative Care/Hospice will be responsible for assessing the needs of patients, communicating with the interdisciplinary care team, and developing plans to help patients and their families cope with the physical and emotional aspects of their illness. The ideal candidate will possess excellent communication skills, a strong work ethic, and the ability to maintain a professional and empathetic demeanor.To be considered for this role, successful applicants will need a Master’s Degree in Social Work with a minimum of two (2) years of experience working in a palliative care and/or hospice setting. A valid Social Worker license in the State of New Mexico is also required. Responsibilities for Outpatient Social Worker Palliative Care/Hospice:
- Assess the needs of patients and their families who are dealing with serious illness or end of life care.
- Communicate with the interdisciplinary care team to develop plans to help patients and their families cope with the physical and emotional aspects of their illness.
- Provide support and guidance to patients and their families in order to ensure they receive quality care with empathy and understanding.
- Possess excellent communication skills and maintain a professional and empathetic demeanor.
- Maintain a strong work ethic and adhere to professional and ethical standards.
- Maintain valid Social Worker license in the State of New Mexico.
Excellent Interpersonal And Communication Skills
Bachelor's Degree In Social Work

Licensed Social Worker (Lsw) In The State Of New Mexico
Two Years Of Experience In Medical Social Work
