
RN - Transplant Coordinator
We are looking for an experienced Registered Nurse to join the Transplant Services team at Presbyterian Healthcare Services as our Transplant Coordinator. The ideal candidate will have a passion for helping others and be able to effectively navigate the complexities of the transplant process. The successful candidate will be responsible for managing all aspects of the transplant process and providing support to transplant patients throughout their journey.The ideal candidate will possess the following qualifications: Bachelor’s Degree in Nursing; current licensure in the State of New Mexico; at least three years of experience in a transplant or surgical setting; and knowledge of EMR systems. In addition, the candidate should have excellent interpersonal and organizational skills, be able to work independently, have strong problem-solving abilities, and be able to handle a fast-paced environment. Responsibilities:
- Provide support to transplant patients and their families throughout the transplant process.
- Manage all aspects of the transplant process.
- Follow established protocols and procedures.
- Document and report patient care activities and outcomes.
- Monitor patient progress and health status.
- Assist with patient education and discharge planning.
- Provide resources to patients and families regarding transplant options and risks.
- Ensure patient safety and compliance with regulations.
- Develop and implement policies and procedures for the transplant program.
- Collaborate with transplant team members and other healthcare professionals.
Bachelor's Degree In Nursing
Current And Valid License As A Registered Nurse In New Mexico
Minimum Of Three Years Of Experience In An Acute Care Setting In An Inpatient Transplant Unit
Ability To Develop And Maintain Relationships With Transplant Patients
Understanding Of Acute And Chronic Care Management Of A Transplant Patient
