
Senior Quality Program Manager-Population Health
We are looking for an experienced healthcare professional to join our Population Health team as Senior Quality Program Manager. In this role, you will have an opportunity to make a meaningful impact on the care and outcomes of our patients. You will be responsible for developing, implementing, and measuring quality improvement initiatives to ensure that our patient population is receiving the highest quality of care.The ideal candidate will have a minimum of 5 years of experience in a healthcare setting as well as a strong working knowledge of quality improvement and population health principles. A successful Senior Quality Program Manager-Population Health at Presbyterian Healthcare Services will have excellent project management and communication skills and be comfortable working collaboratively with clinical and non-clinical staff. Responsibilities:
- Develop, implement, and evaluate quality improvement initiatives related to population health.
- Lead and manage quality improvement projects and initiatives.
- Monitor and analyze clinical data to identify areas of improvement in the patient population.
- Collaborate with clinical and non-clinical staff in order to ensure successful implementation of quality initiatives.
- Draft and maintain detailed reports and documentation to track progress of quality initiatives.
- Participate in relevant meetings and conferences to stay up-to-date on the latest industry trends.
- Identify potential opportunities for improvement and develop strategies to address them.
- Maintain a working knowledge of relevant laws and regulations.
- Lead and mentor team members in the development of quality improvement initiatives.
- Foster relationships with external partners to promote quality improvement.
Five Years Of Experience In A Healthcare Setting
Nursing
Bachelor's Degree In Healthcare
Public Health Or Related Field
Three Years Of Experience In A Quality/Regulatory/Compliance Role
