Current diabetes care and education specialist looking to transition to more of a managerial role in overall and population health. I’ve lived with Type 1 diabetes for 14 years and want to help others with chronic diseases live healthier lives.

•Successfully implemented and manage a Diabetes Prevention Program.
•Provides quality diabetes self-management education and medical nutrition therapy in individual and group settings based on assessed needs. 
• Provides outpatient medical nutrition therapy for diabetes, weight management, renal, and cardiovascular disease
• Follows the ADCES Diabetes Education Accreditation Program requirements which are based on the National Standard of Diabetes Self-Management and Support Guidelines. 
• Integrates skills and knowledge of pathophysiology, epidemiology, clinical management, cardiometabolic conditions, and self-management of diabetes into clinical practice.
• Assist patients in monitoring blood glucose and insulin pump data for the purpose of medication control and adjustment for both insulin and noninsulin dependent individuals. 
• Successfully initiated and currently manage Diabetes Prevention Program. 
• Work alongside providers making recommendations/suggestions for medication changes, titrations, and initiation of new diabetes medications. 
• Certified trainer for Tandem, OmniPod and Medtronicinsulin pumps
• Certified trainer for Dexcom, Medtronic and FreeStyle Libre CGM. 
• Serve as part of the medical staff at Camp Morris, a camp for children living with Type 1 Diabetes
• Actively participates in the quality improvement (QI) process and adapts practice/process based on QI findings.

Provided outpatient medical nutrition therapy for diabetes, weight management, renal, and cardiovascular disease
• Devised meal plans, teach carbohydrate counting and perform nutrition-related functions in addition to pattern management and glucose meter monitoring.
• Educated patients on diabetes self-management skills, such as BG monitoring, insulin actions and hypo/hyperglycemia s/s. 
• Started and successfully managed cooking classes and cooking demonstrations working alongside national grocery store chains to provide food preparation education for individualized health goals.
• Started and successfully managed a monthly diabetes support group.
• Assisted patient’s w/ insulin pump changes made by physician and helped troubleshoot pump related problems with patients using Medtronic 530g, 630gand 670g, OmniPod, and Tandem T-Slim insulin pumps
• Assisted doctors with interpretation of continuousglucose monitor reports (CGM)
• Trained and assisted patients on use of Medtronic CGM, Dexcom, and FreeStyle Libre Sensors
• Instructed patients on insulin injections and glucometer use
• Successfully managed Bluetooth Glucose Monitoring program working with patients and endocrinologist to help patients manage blood glucose from home
• Coordinated with primary care offices within Medical Group to promote referrals to program
• Taught American Diabetes Education certified group education classes
• Tracked diabetes program monthly education hours
• Assisted in yearly recognition data collection required for ADA program certification
