•       Currently responsible for a panel of approximately 700 patients. Providing primary and family evidenced-based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling and care planning.
•       Complete annual well visit exams for Medicare patients to help identify and address potential medical needs that may lead to future complications.
•       Perform duties including speaking with patients about healthy lifestyle choices, completing diagnostic test to help diagnose and treat patients for injuries or illnesses and coordinating with other healthcare professionals to maintain a comprehensive plan of care.
•       Encourage patients to become active participants in their own healthcare and well-being through services such as health screenings, behavioral health and wellness and chronic condition management.
•       Establish care with new patients from the community, retain and build trusting provider-patient relationships that will allow the opportunity of maintaining healthy outcomes.

ARNP

•       Diagnose and treat both acute and chronic conditions such as diabetes, hypertension, mental health disorders, infections or acute exacerbations of chronic diseases.
•       Perform physical examinations, order and interpret any necessary diagnostic testing and offer best treatment plan based on individual needs
•       Complete well woman exams, contraception management and safe sex education
•       Counsel patients on various disease conditions to ensure that they are educated on their disease process, treatment and expected outcomes
•       Work as an integral part of a comprehensive and collaborative team in order to provide quality evidenced-based care to our patient population

•       Sole provider at primary care office where I managed a panel of approximately 1000 individuals from the ages of birth to end of life.
•       Effectively identify, evaluate and address disease prevention and health promotion issues in multiple populations while administering quality patient care.
•       Work in conjunction with multidisciplinary team including physicians, to ensure that the best and most cohesive patient care possible is being implemented and followed.
•       Educate patients and families about preventative care, medical issues and the use of prescribed medical treatments or medications.
