Community Health Worker

San Juan Regional Medical Center is seeking an experienced and compassionate Community Health Worker to join our team. The ideal candidate will have a passion for connecting individuals and families to the healthcare and social services they need to promote health and wellbeing. In this role, you will work directly with patients to assist them in accessing healthcare resources, providing education on health topics, and advocating for their needs.The successful candidate will have knowledge of local healthcare resources, experience in collaborating with community partners, and the ability to build relationships with patients of diverse backgrounds. We are looking for an individual who is patient, empathetic, and able to work with individuals from all walks of life.Qualifications:• Education/experience in social services, health care, or public health• Knowledge of local healthcare resources and community partnerships• Ability to build relationships with patients of various backgrounds• Ability to work independently and as part of a team• Excellent communication and interpersonal skills• Ability to work in a fast-paced environment• Bilingualism in English/Spanish is preferred

Responsibilities: • Provide education and resources to patients regarding health topics and available healthcare services • Assist patients in accessing healthcare resources and social services • Advocate for patient needs and ensure they receive the care they need • Collaborate with community partners to coordinate resources for patients • Build relationships with patients of diverse backgrounds • Monitor patient progress and provide follow-up support as needed • Maintain patient records and update databases as necessary • Provide feedback and recommendations to staff and management • Work independently and as part of a team in a fast-paced environment • Bilingualism in English/Spanish is preferred
