  1. Conduct initial assessments for individuals with disabilities: As a PAS Assessor, you will be responsible for conducting initial assessments to determine the needs and level of support required for individuals with disabilities in the State of Arizona.
  2. Collaborate with service providers: You will work closely with service providers to ensure that individuals with disabilities receive appropriate and quality services.
  3. Maintain accurate records: It is important to maintain accurate and up-to-date records of assessments and services provided to individuals with disabilities.
  4. Conduct follow-up assessments: You will be responsible for conducting follow-up assessments to evaluate the effectiveness of services provided and make any necessary adjustments.
  5. Provide recommendations: Based on assessments, you will provide recommendations for services and support that will best meet the needs of individuals with disabilities.
  6. Communicate with clients and their families: You will be the main point of contact for clients and their families, providing them with information, support, and guidance throughout the assessment process.
  7. Stay updated on policies and regulations: As a PAS Assessor, you will be expected to stay updated on relevant policies and regulations related to services for individuals with disabilities in the State of Arizona.
  8. Monitor service delivery: You will monitor the delivery of services to ensure they align with the recommendations made in assessments.
  9. Identify community resources: You will research and identify community resources and support services that may benefit individuals with disabilities and their families.
  10. Support the mission of the organization: As a member of our team, you will be expected to uphold our mission of enhancing the quality of life for all Arizonans through your work as a PAS Assessor.

Job Qualifications
  • Bachelor's Degree In A Relevant Field Such As Social Work, Psychology, Or Nursing.

  • Minimum Of 2 Years Of Experience Working With Individuals With Disabilities Or Older Adults.

  • Knowledge Of Arizona's Disability And Aging Services Systems, Including Eligibility Criteria And Available Resources.

  • Strong Communication And Interpersonal Skills, With The Ability To Conduct Thorough Assessments And Develop Comprehensive Care Plans.

  • Proficiency In Computer Software And Database Management, As Well As The Ability To Maintain Accurate And Detailed Records.
