
Pre-Employment Transition Coordinator

You will be responsible for developing and implementing individualized transition plans, connecting individuals with potential employers, and providing job coaching and guidance. Responsibilities:
- Develop and implement individualized transition plans for individuals with disabilities.
- Connect individuals with potential employers.
- Provide job coaching and guidance.
- Monitor progress of individuals with disabilities in the workplace.
- Advocate for individuals with disabilities in the workplace.
- Keep up to date on disability services and regulations.
- Facilitate meetings and trainings.
- Maintain records of services provided.
- Collaborate with other professionals to provide comprehensive services.
- Evaluate the success of transition services and make recommendations for improvement.
