  1. Provide case management services to individuals and families in need, including assessing their needs, developing care plans, and connecting them with appropriate resources and services.
  2. Advocate for clients and their families by identifying and addressing barriers to accessing necessary care and services.
  3. Develop and maintain positive and collaborative relationships with community organizations, service providers, and other stakeholders to ensure the best possible outcomes for clients.
  4. Maintain accurate and detailed case records, including progress notes, assessments, and service plans.
  5. Conduct regular follow-up with clients to monitor their progress and make necessary adjustments to their care plans.
  6. Collaborate with interdisciplinary team members, such as healthcare professionals and social service providers, to develop and implement comprehensive care plans for clients.
  7. Stay up-to-date on relevant laws, policies, and procedures related to social work and case management in the State of Texas.
  8. Provide crisis intervention and support to clients and their families as needed.
  9. Communicate effectively with clients and their families, using empathy, active listening, and culturally competent approaches.
  10. Participate in regular supervision, team meetings, and professional development activities to enhance skills and knowledge.
  11. Comply with all confidentiality and ethical standards in handling sensitive client information.
  12. Continuously evaluate and improve the effectiveness of services provided to clients, and make recommendations for program enhancements or changes as needed.
  13. Collaborate with other agencies and organizations to promote awareness and education on social issues and resources available to the community.
  14. Adhere to all relevant state and federal laws, regulations, and agency policies and procedures.

Job Qualifications
  • A Bachelor's Degree In Social Work (Bsw) Or A Related Field Such As Psychology Or Sociology.

  • At Least 2-3 Years Of Experience In Case Management Or Social Work, Preferably In A Government Or Non-Profit Setting.

  • Knowledge And Understanding Of State And Federal Laws, Regulations, And Policies Related To Social Work And Case Management.

  • Strong Communication, Organizational, And Problem-Solving Skills.

  • A Valid State Social Work License And/Or Certification As A Certified Case Manager (Ccm) Or Certified Social Worker (Csw) Is Preferred.
