  1. Conduct follow-up services for individuals who have been exposed to communicable diseases, including contacting them and providing guidance on necessary steps for treatment and prevention.
  2. Utilize knowledge of public health principles and practices to assess the potential risk and impact of communicable diseases on the community.
  3. Maintain accurate records of all follow-up activities and update relevant databases and systems.
  4. Collaborate with other healthcare professionals and agencies to ensure effective and efficient communication and coordination of services.
  5. Stay updated on current public health guidelines and protocols for handling and preventing communicable diseases.
  6. Educate and inform individuals and the community about communicable diseases, prevention methods, and available resources.
  7. Develop and implement strategies to reach and engage high-risk populations in follow-up services.
  8. Provide support and assistance to individuals and families during the follow-up process, including addressing any concerns or questions they may have.
  9. Participate in training and professional development opportunities to enhance knowledge and skills related to public health and communicable diseases.
  10. Represent the organization and communicate effectively with stakeholders, including government agencies, healthcare providers, and community organizations.
  11. Adhere to all confidentiality and ethical standards when handling sensitive information.
  12. Monitor and report on the progress and success of follow-up services, identifying areas for improvement and making recommendations for program enhancements.
  13. Act as a resource and consultant for colleagues and other professionals on public health and communicable disease related matters.
  14. Adhere to all state and federal regulations and policies related to public health and communicable diseases.
  15. Continuously evaluate and adapt follow-up procedures and protocols to ensure the most effective and efficient services are provided to the community.

Job Qualifications
  • Bachelor's Degree In Public Health Or A Related Field

  • Minimum Of 3 Years Of Experience In Public Health Follow Up Or Disease Surveillance

  • Knowledge Of State And Federal Laws And Regulations Related To Public Health

  • Excellent Communication And Interpersonal Skills

  • Ability To Work Independently And As Part Of A Team In A Fast-Paced And High-Pressure Environment.
