  • Collect, analyze, and report data on community health indicators to inform decision-making and measure program effectiveness

  • Facilitate workshops, trainings, and other educational events on various health topics
  • Participate in community meetings and events to represent the organization and promote health initiatives
  • Maintain accurate records and documentation of program activities and outcomes
  • Stay informed about current public health trends, policies, and regulations to inform program development and implementation
  • Collaborate with other members of the health care team to ensure comprehensive and coordinated care for community members.

Job Qualifications
  • Bachelor's Degree In Public Health, Community Health, Or A Related Field.

  • At Least 2 Years Of Experience Working In A Community Health Setting, Preferably With Underserved Populations.

  • Knowledge Of Community Health Principles, Program Development, And Evaluation.

  • Strong Communication And Interpersonal Skills, With The Ability To Effectively Engage And Educate Diverse Populations.

  • Proficiency In Data Collection, Analysis, And Reporting, With Experience Working With Electronic Health Records And Other Health Information Systems.
